Kindergarten Registration

Kindergarten registration for the 2009 – 2009 school year is scheduled for Wednesday, April 9 and Wednesday, April 23. Ashland Elementary will be open from 10:00am To 12:00pm and 1:00pm to 3:00pm for registration. Information can be found on the School Division Web site at http://www.pwcs.edu/schools/kindergarten/kindergarten.html . 

You must have the following to register a child:
    Original State Birth Certificate
    Social Security Card
    Physical

Immunizations
    3 OPV 1 after 4th B-Day
    3 DTP 1 after 4th B-Day
    2 MMR 1 after 1st B-Day
    3 Hepatitis B
    Varicella after 1st B-Day
    Or proof of Chicken Pox

You must have at least one of the following:
Lease, Mortgage Papers, or Affidavit

Plus two of the following:
Drivers license, Utility bill, Car Reg., Voter Reg. Card, Tax bill, County Sticker receipt.
